The application makes it possible to define social needs and functional assessment of persons with disabilities. This application is composed of electronic questionnaires and includes the final assessment and score count system.
Case Study
Application consists of the following instruments:
- Functional assessment instruments - to determine how the human health condition affects various areas of its functioning: Self-care, communication, movement, memory, perception, focus on concentration, daily activities, education, employment, participation in public life, etc.
- Social needs assessment instruments - identifies the needs of the child or adult person, care, the use of auxiliaries, the needs of social and medical services.
